Basket case 意為「極度緊張 (或焦慮) 的人；精神瀕於崩潰的人；財政拮据的公司、組織或國家」，其中 case 的原意為「病人」。最初這個複合名詞是指完全傷殘 (或不能行動) 的人。
最早提到 basket case 的出版品是紐約《雪城先鋒報》(The Syracuse Herald)，該報在一次世界大戰結束後的翌年，即 1919 年，將此一名詞定義為「失去四肢，必須用籃子 (basket) 揹著走的士兵」。到了 1921 年，這名詞開始出現上述的比喻意思。它的複數為 basket cases。
